Address-Phone Number-Name Change

LSCPA communicates by email with students. It is the responsibility of students to maintain current contact information in Banner.

Students who move must complete a Change of Address form and deliver it to the Records Office. Change of Address forms are located at the Office of Admissions and Records third floor of the Student Center. Students may also update their address or phone number using their assigned MyLSCPA email address with an email sent directly to admissions@lamarpa.edu.

Change of name because of marriage or correction of name because of spelling errors may be made by completing a Name Change Form at the Office of Admission and Records. All name changes must be accompanied by a copy of the legal document making the name change official, such as a marriage certificate or signed social security card.

Students who wish to make changes to gender must provide a copy of the legal documentation that indicates the change being made.
